Payment is to be rendered at the time of treatment. We accept cash, checks, Visa and MasterCard.

 

As a convenience to you, we will assist in processing your insurance. Remember that you, and not your insurance carrier, are responsible for payment of services rendered in our office.

 

Effective June 1, 2020, we no longer participate in Medicaid. 
We are also not accepting new Rite Smile patients.  If there are any questions, please call our office.
